Meet Our Team
MEET OUR TEAM
Sofia Carreon first joined the UCP Downtown Clinic in November 2017 as Bilingual Receptionist and Community Outreach Specialist. She enjoys greeting families and seeing the children running to start their therapy sessions. Before UCP, Sofia interned at ASU Prep Academy where she worked directly with children in a second grade class, assisting them with their school work, reading groups and worked closely with those students who needed an extra push.
Sofia is currently an undergrad student at Arizona State University working on her Bachelors of Science in Health Care Delivery and minoring in Community Health and is a proud member of Sigma Kappa sorority. Her career goal is to hold an administrative position at a children’s hospital or nonprofit health care organization working with children. On her down time, Sofia enjoys spending time with friends and long walks at the beach.
HCBS Program Coordinator
Tammy Ellis joined the UCP team in October, 2003 as a Direct Care Worker, then quickly shifted to Program Coordinator. Tammy fulfills her passion of helping families and the members we serve in her role. For the past 27 years, Tammy has been working and volunteering in the disability community somehow, someway! Her extended years of experience in so many various areas help her perform her job in the best way possible.
Doing the right thing is her motto. Tammy says “If you do the right thing, everything else will fall into place.” Tammy and her husband of 35 years have one adult daughter and three grandchildren. Tammy is originally from Michigan but considers herself an Arizonan since she’s lived here twice as long as she lived in Michigan. Her favorite thing to do is spend time with her family.
Delia Chavez joined the UCP team in July, 2016 as the Bi-lingual Program Coordinator. Her position allows her to serve the community with firsthand experience, as she is the mother of a child with a disability. She strives to reach out to families, members and our providers in order to provide an environment that promotes trust by addressing the needs of everyone involved.
Delia has an AA in Psychology and has continued her education at the University of Texas of El Paso. She is also a certified CPR, First Aid and Article 9 instructor and has been working in the Home Base Community Services for over 7 years. Her favorite past times include reading, binge watching Netflix and spending time with her family. Delia has been married for 32 years, has two adult children and two pets. She is originally from El Paso, Texas and has lived in Arizona for the last 16 years.
Clinic Therapy Manager
Laura Hazy first joined the UCP team in July 2015 as the Clinic Therapy Manager for both The Laura Dozer Clinic in North Phoenix, and recently the Downtown Clinic. She loves being the first point of contact for families looking to help their children receive high quality therapy services. Before UCP, Laura worked directly with children with disabilities and behavioral challenges as a direct care provider, behavioral tech, and also as a clinical supervisor. “The best part is seeing children make progress! And of course supporting the talented therapists who make a difference in the lives of children every day”, says Laura.
Laura holds a Bachelors of Arts is Psychology, and she is working on her Master of Science in Family and Human Development. Laura dedicates her time outside of work to her wonderful family, including her Fiancé, son, and 2 dogs.
Brittany McFadden joined the UCP Downtown team in December 2016. She enjoys collaborating with each child and family to support their unique development and potential. Brittany previously worked with UCP in our early intervention program and returns to Phoenix from working internationally in health care.
Brittany holds a B.S in Speech and Hearing Science from ASU and an M.S. in Occupational Therapy from the University of Puget Sound. Outside of work, Brittany is pursuing an MA in Theology and enjoys collaborating with local organizations to encourage positive inter-cultural relationships. She spends her free time with friends, hiking and reading.
Marnie Madden joined the UCP team in February 2016, as our well respected feeding therapist. Marnie has many years of experience in evaluating and treating children with feeding disorders related to complex medical issues, sensory processing disorder and/or behavioral difficulties. She has worked as a feeding therapist in neonatal and pediatric intensive care units, families’ homes and in out-patient clinics, similar to UCP Downtown.
Lauren Ellish joined the UCP team as a speech-language pathologist in October 2017. Lauren comes to UCP excited to provide individualized therapy in the clinic setting where she can build strong, trusted relationships with families. Lauren is passionate about helping each child unlock opportunities and potential through access to communication. She takes a holistic and diagnostic approach to communication evaluations and therapy. Lauren enjoys working with children with a wide range of speech and language needs, including special interests in speech sound disorders, social pragmatic language disorders, and links between language and literacy.
Lauren earned her Master of Science Degree in Clinical Speech-Language Pathology from Purdue University and her Master of Business Administration and Bachelor of Science degrees from Cornell University. She holds the Certificate of Clinical Competency (CCC-SLP) from the American Speech, Language, and Hearing Association (ASHA). Lauren previously worked as a speech-language pathologist in the school setting providing therapy for children in preschool through eighth grade.
In her free time, Lauren enjoys spending time with her family, playing board games, hiking, cooking, and taking road trips.
